The Name Formality tab page on the Local Preferences dialog allows you to determine the formality of names printed on various output forms. Formal names use First and Last Name, and Informal names use Goes By and Last Name.

The Name Formality dialog gives you the option of including Middle Name/Initial and/or Suffix for formal names. Check the options you wish to include in the printed name if available.

Formality for Output Forms

Click on the preferred formality for each output form that uses names. These preferences can be changed at any time if you need to change the formality of the names for a specific report you wish to print.

However, CMD will remember your choices here and will always print the various forms with your choice of formality unless you change it on this dialog.
