To print a report containing only name and phone numbers, click the Name & Phone item on the Output Forms combo box on the Print Control page of the Print Names dialog.

The additional choices to be made for this output form is whether or not you wish to print just home, work, and cell phone numbers, or all the phone numbers that apply.

You may optionally also include e-mail addresses by clicking the Include E-Mail Address check box.

When you elect to Include the Spouse in the report, the spouse's work number or other phone numbers will be printed as well.
